You will be asked to fill out a very brief medical history form. 

Our medical staff will check your vital signs, take a brief history, and assess your immediate medical needs.  Hyper-acute cases are referred to the hospital emergency room.  Cases requiring other and/or continuing care are referred through our existing health care system.  Some brief, limited care is usually available at the clinic.

Our patient advocate will interview you to be sure that you get any help we can give you in order to comply with our physician’s assessment of your needs.
